A Unique Program

A life-changing brain injury can happen in an instant. Often, this results in a visit to the emergency room. Following the emergency room, many patients receive care and rehabilitation in an acute hospital. Although initial therapy is important, recovery does not end with inpatient rehabilitation. In fact, 40% of all patients hospitalized with traumatic or acquired brain injuries are discharged with unmet needs. Mentis addresses those needs and continues rehabilitation in a realistic home-like setting to complete the transition back home and to work whenever possible. Completing rehabilitation in this specialized setting helps develop the skills necessary for long-term success.

Admission Criteria

Mentis Neuro Rehabilitation is a post-acute, residential rehabilitation facility serving persons with acquired brain injury. The admission criteria include:
  • Seventeen years of age or older.
  • History of neurological or neurobehavioral disorder.
  • Medically stable (not requiring complicated medical intervention).


Referral Process

Mentis Admissions Coordinators assist individuals, family members, physicians, social workers, case managers, discharge planners and claims representatives for a smooth transition into one of our facilities. Our Admissions Coordinators will acquire all relevant information and schedule a pre-admission evaluation by one of our Clinical Evaluators. The evaluator will create a thorough report including recommendations that are based on the Mentis Neuro Rehabiltation admission criteria.
